Sacramental Certificate Request

If you require a certificate for a Sacrament that took place at St. Anne Parish, please email the following information to:  stanne@stanneparish.org.

• Sacrament Certificate requested (Baptism, First Communion, Confirmation, Marriage)
• Name of person who received the Sacrament
• Name and relationship of the person requesting the certificate
• Date of birth of person who received the Sacrament
• Date (or approximate date) Sacrament was received
• Email address to send certificate
• Address to mail certificate (Please note that Baptism certificates requested for Marriage must have a parish seal and must be mailed.)
